How to keep sugar fresh

If your brown sugar has gone hard, add a piece of bread into the container. I have no idea how it works, but it keeps the sugar fresh.

A friend has told me to keep a slice of apple in the sugar. I did (but I over did it as I added two slices) and it was great, the sugar loosened up. However, I left the apple slices in the sugar for more than one week, and it turned moldy. I had to throw the whole jar of sugar away. Do I have to keep on changing the piece of bread regularly to keep the sugar (and the bread) from molding and if yes how often? Thanks!
